Guardian PPO Dental Provider Network

You become eligible to join either the Value or NAP Guardian PPO Dental Plans on the 1st of the month following 90 days of employment. To help make your selection between Value or NAP (rates are the same) please click here. If you do not elect to participate in the Guardian Dental Plan following your waiting period, you will not be eligible to join again until open enrollment (month on October) with coverage becoming effective on November 1st. |
